On this order form, put
the number of donuts you want by each kind.
Example:
if you want
6 chocolate and 6 sugar & cinnamon donuts, put a 6 in the box in
front of "Chocolate
Frosted" and a 6 in the box in front of "Sugar & Cinnamon".
12 is your Total Count and Total Due is $7.50.
We will double check your
order to be sure you'll get exactly what you want.
Be sure to put your
name at the bottom so we can call you to the window when your order is
ready. This is really important on our busy weekends!
